In most cases, we can refer a caregiver within 24 hours of completing an in-home assessment. The intake process starts with a call to one of our care coordinators, and because our team already has working relationships with discharge planners at NYU Winthrop Hospital in Mineola and Nassau University Medical Center in East Meadow, we're often able to coordinate the transition before your loved one even leaves the hospital. The goal is to make sure care is in place the day they come home, not a week later when gaps have already created problems.

It depends on what the patient needs, which is why we start with a nurse-developed care plan rather than a generic service list. For stroke patients, daily support typically includes help with bathing, dressing, and mobility; medication reminders on a set schedule; meal preparation that accounts for any dietary restrictions related to stroke recovery; transportation to outpatient therapy or physician appointments; and reinforcement of exercises or routines established by a physical or occupational therapist. Our caregivers are also trained to recognize warning signs that may indicate a change in the patient's condition and to escalate those concerns to the supervising RN promptly.

Coverage depends on the type of care needed and the patient's specific insurance situation. Medicare may cover skilled home health services, like nursing visits or physical therapy, when certain conditions are met following a hospital stay. For non-skilled personal care and companion support, coverage often comes through Medicaid Managed Long Term Care plans, of which we have established relationships with contracted MLTC plans serving Nassau County. We also work with private pay arrangements. Our care coordinators will help you understand exactly what's covered and what isn't before you commit to anything, we don't hand you a bill and leave you to figure out the insurance side on your own.
