Referring a patient is a single conversation.

Axzons accepts referrals from hospitals, physicians, discharge planners, social workers, and other healthcare professionals. We handle the coordination so the family does not have to.

We coordinate the transition

A care coordinator reviews the referral the same day during business hours, confirms service availability, and schedules a free in-home assessment.

How does a hospital, physician, or discharge planner refer a patient?

Send patient details through our Request Care form or call intake at (866) 4AXZONS. A care coordinator reviews the referral the same day during business hours, confirms service availability, and schedules a free in-home assessment with a licensed nurse.

Does Axzons work with Healthfirst or CenterLight (PACE / MLTC) members?

Yes. Where plan benefits and our service areas align, Axzons coordinates homecare for Healthfirst members and CenterLight PACE participants, working with their care teams so services and authorizations stay in step. Coverage varies by plan, eligibility, and authorization. A coordinator confirms specifics on the first call.

How do families pay for care?

Axzons works with Medicaid (including MLTC plans), Medicare Advantage, long-term-care insurance, and private pay. Through our banking relationship with JPMorgan Chase, private-pay families have secure, flexible payment options. What applies to a given family depends on state, plan, eligibility, and authorization, and a coordinator reviews it before care begins.

Is Axzons Homecare accredited and regulated?

Axzons holds The Joint Commission's Gold Seal of Approval and is licensed and regulated by the state health department in each of the six states it serves. That oversight is one reason referring providers can hand off with confidence.

A care coordinator calls

Every call is handled the same day during business hours. The coordinator who calls is the one who stays with your case.

We talk through the situation

Needs, schedule, location, and any insurance questions are mapped on the first call, in plain language.

Step 04

Free in-home assessment

If you'd like, one of our Registered Nurses can visit at no charge to see the home and meet the person in care.

Step 05

Caregiver matching begins

Where caregivers are appropriate, we propose one to three options that best serve the client. The family can pick who they find most suitable.

Step 06

Care continues, with one number

Plans are reviewed monthly by our nursing team and adjusted as life changes. The same coordinator picks up the phone.

Final coverage and eligibility must be confirmed directly with the care coordinator on the first call.
