Safe, professional infusion therapy — at home, where it makes sense.
In-home infusion support across Nassau, Suffolk, Queens, and Westchester, where clinically appropriate and physician-prescribed. Skilled nurses monitor patient response and coordinate with your healthcare providers.
$249 / visit to start · complex infusions priced after clinical review

Physician-prescribed and availability-dependent. In-home infusion is not offered for every therapy or every patient. Availability depends on a physician order, clinical assessment, payer requirements, pharmacy coordination, applicable state rules, and the service area. A care coordinator confirms what applies before any commitment.
A convenient alternative to frequent clinic visits, where appropriate.
Patients managing chronic conditions on ongoing, physician-prescribed therapies
Patients recovering from illness or transitioning home after a hospital stay
Individuals for whom frequent clinic visits are impractical, where home infusion is appropriate
Families who want skilled nurses to coordinate with the physician and pharmacy
Nurse-led infusion support, end to end.
RN visit (1.5–3 hours)
A registered nurse on site for the duration the therapy requires — typically 1.5 to 3 hours depending on the infusion.
IV insertion & monitoring
IV access, line and site monitoring, and observation of patient response throughout the visit.
Pre-infusion assessment
A pre-infusion vitals and history check before any physician-prescribed infusion is administered.
Post-infusion monitoring
Post-infusion monitoring and documentation, with escalation to the physician by phone when something changes.
Physician-prescribed administration
Infusions administered only as prescribed, by a licensed nurse, where state scope of practice allows.
Physician & pharmacy coordination
Coordination with the prescribing physician and specialty pharmacy so the family isn't chasing the pieces.
Infusions we support, depending on the prescription.
- Hydration IV therapy
- Vitamin infusions for wellness-focused clients, where appropriate
- Antibiotic therapy for post-hospital patients
- Specialty medication infusions
Availability of any specific therapy depends on the prescription, clinical appropriateness, and authorization.
A schedule that follows the care plan.
- Typically 1–3 visits per week, depending on condition and prescription
- Short-term post-hospital support, often 1–2 weeks where appropriate
- Long-term chronic therapy, ongoing weekly where appropriate
- Complex infusions may require separate pricing and clinical review
How an Axzons infusion arrangement starts.
Physician order on file
Infusion services begin only after a prescribing physician's orders are received and reviewed by our nursing team.
Nurse-led assessment
An RN reviews the household, the line/access site, and the family's schedule before any infusion is scheduled.
Coordinated delivery
We coordinate with the specialty pharmacy and prescribing physician, then nurses deliver according to the plan.
Infusion services require a physician order and clinical review. Axzons does not provide emergency infusion services or hospital-level emergency care at home. For medical emergencies, call 911.
