St. Francis Hospital & Heart Center is New York State's only specialty cardiac hospital, and the patients coming home from there often have recovery needs that go well beyond what a standard home health aide is equipped to handle. Post-cardiac recovery typically involves complex medication regimens — blood thinners, diuretics, beta blockers — dietary restrictions like sodium-limited cardiac diets, activity limitations, and wound monitoring if the patient had open-heart surgery or a catheterization procedure.
We already serve Flower Hill and Manhasset, both within the Town of North Hempstead and close to the St. Francis campus. A licensed nurse reviews the discharge instructions from St. Francis and builds a care plan around the specific clinical picture — not a generic checklist.
