The role supports patient-census growth, relationships with the Hispanic community, and clear communication with patients, families, and caregivers.

Day to day

Responsibilities

  • Serve as the liaison between patients, families, caregivers, and the agency.
  • Help grow patient census through referrals, community visits, and relationships with Hispanic community leaders, families, and organizations.
  • Communicate actively, clearly, and warmly with patients, caregivers, and agency staff.
  • Make calls, follow up, and coordinate services.
  • Assist with the intake process for new patients.
  • Maintain accurate, organized records.
  • Use computers, internal systems, and agency software efficiently.
  • Identify growth opportunities to expand Axzons' presence in the community.
  • Support office operations as needed.

Qualifications & Skills

  • Fluent in spoken and written Spanish (required).
  • Basic/intermediate English communication, spoken and written (required).
  • Excellent communication and people skills.
  • Sociable, conversational, and relationship-oriented.
  • Strong focus on goals, organization, and patient-census growth.
  • Able to work independently and as part of a team.
  • Comfortable with computers: email, document handling, and internal systems.
  • Prior homecare, patient coordination, or customer service experience preferred but not required.
