Axzons Homecare is seeking a Registered Nurse (RN) – Director of Patient Services (DPS) to oversee clinical operations while actively contributing to the growth of patient census in New Jersey. This role blends clinical leadership, regulatory compliance, care coordination, and business-development support to ensure high-quality patient care and sustainable agency growth. The DPS supervises clinical staff, ensures compliance with NJ Division of Consumer Affairs and DOH regulations, develops referral relationships, and supports initiatives that increase patient admissions while maintaining excellent clinical outcomes.
Responsibilities
- Clinical oversight: serve as the primary clinical authority for Axzons Homecare NJ operations.
- Ensure compliance with New Jersey home care regulations, agency policies, and payer requirements.
- Review, approve, and update patient care plans and clinical documentation.
- Conduct initial and supervisory assessments for clients as required.
- Oversee quality assurance, audits, and corrective action plans.
- Supervise and support RNs, LPNs (if applicable), HHAs, and PCAs.
- Census growth: participate in patient-census growth initiatives.
- Build relationships with physicians, hospitals, discharge planners, rehabilitation centers, and community organizations.
- Support intake, assessment prioritization, and timely admission of new patients.
- Collaborate with marketing, intake, and administrative teams to convert referrals into active cases.
- Identify service gaps and recommend new programs or care offerings.
- Operations: coordinate patient assignments to ensure appropriate staffing coverage.
- Train, mentor, and evaluate clinical staff; participate in hiring and clinical onboarding.
- Address escalated patient, caregiver, and family concerns; support retention strategies.
- Reporting: align clinical performance with business goals alongside the Administrator.
- Track and report patient-census trends, admission conversion rates, and quality/compliance metrics.
- Assist with state surveys, audits, and inspections.
Qualifications & Skills
- Active Registered Nurse (RN) license in the State of New Jersey.
- Minimum 2–3 years of home care or home health experience.
- Prior experience as Director of Patient Services, Clinical Manager, or Lead RN preferred.
- Strong knowledge of NJ home care regulations and Medicaid/private-pay workflows.
- Excellent interpersonal, leadership, and organizational skills.
- Ability to balance clinical quality with growth-oriented objectives.
- Preferred: experience supporting patient-census growth or referral development.
- Preferred: familiarity with EVV systems and home care software.
- Preferred: experience working with multidisciplinary teams.
