About the Job
At Axzons Homecare, we are a team of compassionate, dedicated, and trusted home health care providers managed by healthcare professionals. We are looking for professional, experienced Home Health Aides (HHA/PCA) for in-person cases in Riverhead, NY. English/Spanish-speaking aides are needed. Some cases may have client-specific caregiver preferences, which are handled in accordance with applicable law and agency policy. Personal care services may include assistance with nutrition and meal preparation, light household tasks, basic personal care, and medication reminders.
Day to day
Responsibilities
- Assist patients with activities of daily living, including bathing, grooming, and dressing.
- Provide companionship and emotional support.
- Remind patients about medications under the direction of healthcare professionals.
- Perform light housekeeping and meal preparation.
- Assist with nutrition and diet activities such as shopping and meal preparation.
- Help with bathroom/bedpan routines, walking, and transfers (bed to chair or wheelchair).
- Document patient care accurately and on time.
- Follow HIPAA guidelines and maintain patient confidentiality.
- Support medical documentation and record keeping.
What we look for
Qualifications & Skills
- Valid, current HHA/PCA certification.
- Authorized to work in the USA.
- Two professional references.
- Current annual physical with updated immunizations.
- Prior caregiving or home health aide experience required.
- Familiarity with homecare or similar healthcare settings.
- Able to lift and move patients safely and use a Hoyer lift.
- Experience caring for individuals with developmental disabilities is a plus.
- Zero-tolerance policy: no smoking, drinking, or addictive consumables in a patient's home.
- Zero-tolerance policy: no personal phone use while working, except an emergency that must be explained.
- Zero-tolerance policy: no unprofessional behavior toward patients or Axzons staff.
